What Is Saliva

These glands are located within the mouth: the smallest are the sublingual glands which, as the name suggests, are positioned beneath the tongue; the glands responsible for the greatest saliva production (70%) are the submandibular glands, located at the base of the tongue and at the back of the jaw. Finally, the parotid glands — the largest in terms of volume — produce approximately 25% of total saliva and are located within the cheeks. The secretion they produce, being of a liquid consistency, is composed of 99% water. The remaining 1% comprises various substances, including mineral salts, proteins and glycoproteins, immunoglobulins, and enzymes.

Together, these components work in synergy to carry out the many functions of saliva. But what exactly does saliva do? Here are its main roles:

Protection for the Mouth and Teeth: thanks to its content of minerals and especially bicarbonate, saliva acts as a natural antacid, reducing the buildup of bacterial plaque on the teeth and thereby lowering the risk of cavities. More broadly, it helps create an environment unfavorable to the growth of microorganisms on the mucosa, contributing to the protection of overall health.

Swallowing and Digestion: as the saying goes, digestion begins in the mouth, thanks to the enzymes present in saliva. The combined action of chewing and saliva forms the food bolus, making it easier to swallow. Saliva’s lubricating effect also softens even the hardest foods, reducing the risk of scratches and injuries to the oral mucosa.

What Happens When Saliva Isn’t Enough?

Spicy or very hot foods can cause short-term inflammation that usually resolves within minutes or a few days. Acidic foods such as kiwi, pineapple, citrus fruits, or tomatoes may irritate the mucosa. Smoking is another common factor that can negatively affect the mouth.

Who Is Affected

Sjogren’s syndrome can affect anyone at any age, but it is most often diagnosed in people over 40. Women are particularly at risk, being nine times more likely than men to develop the condition.

After rheumatoid arthritis and systemic lupus erythematosus, Sjogren’s is the most common autoimmune rheumatic disease, affecting roughly 0.4-0.6% of the population.

Risk factors include having a first-degree relative with an autoimmune disease or having an autoimmune condition yourself. As we have seen, Sjögren’s Syndrome makes maintaining proper oral hygiene absolutely essential.
