Tongue candidiasis: how to recognize and treat the infection

Candida albicans is a parasitic fungus that naturally colonizes our skin and the mucous membranes of the mouth. Under normal circumstances, it doesn’t cause any discomfort because our immune system keeps its growth under control, maintaining a delicate yet effective balance among the many microorganisms that live within our bodies. However, when this balance is disrupted, Candida can overgrow and lead to bothersome symptoms. Fortunately, oral candidiasis (also known as thrush) is rarely a dangerous condition, though it can certainly be a source of significant discomfort. Anyone can develop a Candida infection, but – as we will see – some groups of people are more susceptible than others. Let’s take a closer look at what oral thrush is, what causes it, and the treatments available to resolve the condition.

Risk Factors

Although oral Candida is a very common condition, some groups of people are more likely to develop the infection.

Specifically, individuals with a compromised or immature immune system may have greater difficulty maintaining a balance in the oral bacterial flora and counteracting the proliferation of the fungus. This group includes infants, who have yet to fully develop their immune system, as well as the elderly, along with individuals affected by HIV or those taking immunosuppressant medications (such as chemotherapy and radiotherapy drugs). Other medications, such as corticosteroids and antibiotics, also interfere with the immune system and — above all — with the bacterial flora, making candida infection more likely.

Certain medical conditions, such as diabetes, can also affect oral health. In uncontrolled diabetes, saliva contains high concentrations of sugar – the preferred food of Candida. Without effective or properly followed treatment, oral thrush can easily develop.

Candida albicans is not only responsible for oral thrush but can also cause vaginal candidiasis. During childbirth, the microorganism can be transmitted from mother to newborn, making the infant susceptible to infection from birth.

Finally, any oral disorders or habits that lead to dry mouth – such as Sjögren’s syndrome or the consumption of alcohol and tobacco – create a favorable environment for the development of oral thrush.

Symptoms of Tongue Thrush

It can be difficult to notice the first signs of oral thrush, but once discomfort appears, the infection is hard to ignore. Candida causes white, milky lesions on the mucous membranes of the mouth, primarily on the tongue and the inner cheeks. These lesions can be very painful and inflamed, often raised and surrounded by a bright red halo. Thrush patches should not be touched or scraped, as this can lead to bleeding and worsen the inflammation.

Special attention is needed in cases of thrush in infants, which is very common: the lesions are often mistaken for milk spots. Trying to wipe them away can make the condition worse.

In general, the infection is associated with dry mouth, which is also one of its main symptoms. Oral candidiasis produces a distinct feeling of a coated or sticky mouth, intensifying discomfort and the burning sensation.

Thrush on the tongue can negatively affect taste buds. Increased oral dryness, combined with widespread inflammation, can alter the sense of taste.

In people who wear dentures, prosthetics, or orthodontic appliances, thrush can worsen and lead to the development of stomatitis.

Although these symptoms are unpleasant, they usually do not pose a serious health risk. However, in some cases, thrush can spread beyond the oral cavity: in immunocompromised individuals – such as those with HIV, autoimmune diseases, or undergoing cancer treatments – Candida can reach the esophagus. In such cases, the previously mentioned symptoms may be accompanied by a tight feeling in the throat and significant difficulty swallowing.

As we have already noted, oral thrush is a very common condition among infants. In addition to mucosal lesions, in children not yet able to explain the source of their discomfort, we may notice a sudden deterioration in mood, with inconsolable crying and irritability. Infants may refuse the breast or the bottle; while it is not a serious illness, in very young children it is important to pay particular attention to the first signs of discomfort, as the risk of dehydration is very real. Breastfeeding mothers may also become infected with oral candidiasis, which can affect the nipple as well, causing sharp, pin-prick-like pains, flaking, and deep breast pain.

Why is this so fundamental? Because an acidic mouth is not a healthy mouth: teeth may demineralise, and harmful microorganisms may proliferate, causing troublesome and often very painful conditions such as cavities or oral thrush.

What Happens When We Don’t Have Enough Saliva?

We have seen how important saliva is to our health. When the salivary glands do not function as they should and fail to secrete sufficient saliva, we develop a condition known as xerostomia, or dry mouth. This is a widespread condition, affecting approximately 20% of the population. Women — particularly with the onset of menopause — are more frequently affected, but xerostomia does not spare men either.
The causes are varied, ranging from advancing age — with the consequent slowing of salivary gland function — to the use of certain medications, such as chemotherapy drugs, but also many more commonly used ones, such as antihistamines. Some diseases may also present xerostomia among their various symptoms.

The reduction in saliva production can cause difficulties in swallowing and chewing, alter the sense of taste, and lead to damage to the teeth and gums.

Preventing or alleviating xerostomia is sometimes possible with a few simple measures: limiting alcohol consumption, avoiding smoking, and drinking plenty of water are all strategies that benefit our health — and not just that of our mouth! Whether because xerostomia has become a chronic problem, or because — following specific medical treatments such as radiotherapy or chemotherapy for head or neck tumours — the salivary glands have permanently ceased to function, the use of a saliva substitute may become necessary.

In general, dry mouth can be caused by reduced saliva production by the salivary glands or by changes in the composition of the saliva itself. In the case of diabetes, both factors contribute to the discomfort affecting the oral mucosa.

What’s the connection between diabetes and dry mouth?

Dry mouth is a very common condition, affecting up to one-third of the population. However, in people with type 1 and type 2 diabetes, the incidence of xerostomia increases significantly, reaching as high as 50% of diabetic patients.

In the case of individuals affected by diabetes, a true vicious cycle develops: high blood sugar levels lead to an equally high concentration of glucose in the saliva. The harmful microorganisms that naturally inhabit the mouth — and are normally kept under control by the immune system and the action of saliva — feed precisely on sugars. Faced with a particularly abundant feast, they therefore thrive, leading to conditions such as stomatitis or oral thrush. Furthermore, the presence of these pathogenic microorganisms acidifies the oral cavity; in the absence of the correct buffering action of saliva, this acidity damages tooth enamel.

There are several causes behind dry mouth in people with diabetes, and all should be considered as part of proper disease management.

Patients with diabetes are more susceptible to dehydration. In the presence of this disease, glucose — a type of sugar — tends to accumulate in the blood. The kidneys are therefore forced to work overtime to filter and absorb this excess sugar. If diabetes is not correctly managed with the appropriate therapy, the kidneys may struggle to perform this task: the excess glucose is then expelled together with urine, drawing further fluids from the tissues through osmosis. This leads to dehydration, which is further exacerbated by the tendency towards more frequent urination in diabetic patients.

This overload on the kidneys can, over time, compromise their function. One of the most frequent complications of diabetes is in fact progressive renal insufficiency. If left untreated, renal insufficiency can result in severe kidney failure; even in the early stages, however, the condition leads to difficulties in the management of bodily fluids and, as a side effect, the sensation of dry mouth.

Another major warning sign – and one of the most serious conditions linked to diabetes – is elevated blood sugar levels. Hyperglycemia carries several risks, including long-term vision loss. But even at earlier stages, excess blood sugar can translate into a high concentration of glucose in the saliva as well. This consequently makes people with uncontrolled diabetes more susceptible both to xerostomia as a symptom of hyperglycaemia, and to diseases that count dry mouth among their symptoms. Oral candidiasis or oral thrush, for example, thrives in sugar-rich environments… and for people with diabetes, this can be a matter of considerable concern!

Finally, some medications used to treat diabetes may also list dry mouth as a side effect.

How to Relieve Dry Mouth When You Have Diabetes

These glands are located within the mouth: the smallest are the sublingual glands which, as the name suggests, are positioned beneath the tongue; the glands responsible for the greatest saliva production (70%) are the submandibular glands, located at the base of the tongue and at the back of the jaw. Finally, the parotid glands — the largest in terms of volume — produce approximately 25% of total saliva and are located within the cheeks. The secretion they produce, being of a liquid consistency, is composed of 99% water. The remaining 1% comprises various substances, including mineral salts, proteins and glycoproteins, immunoglobulins, and enzymes.

Together, these components work in synergy to carry out the many functions of saliva. But what exactly does saliva do? Here are its main roles:

Protection for the Mouth and Teeth: thanks to its content of minerals and especially bicarbonate, saliva acts as a natural antacid, reducing the buildup of bacterial plaque on the teeth and thereby lowering the risk of cavities.
